A road where speeding is strictly enforced, very few cars do over 30mph it is a perfectly straight road and visibility is 100 % for both pedestrians and drivers.

This image shows a crashed vehicle within the approach of a pedestrian crossing well signed by hazard zig zag lines all in a good state of repair.
Most drivers would already approach a well signed pedestrian crossing cautiously expecting quite naturally the real chance of a pedestrian might be using the crossing, and therefore driving at a speed where you could quite easily stop.

Other than vehicle failure what other reason can there possibly be other than driver error?

Perhaps the need for a road train along the seafront in the distant future.

For reference the lady driving suffered a major heat attack and fell unconscious. Fortunately no one was on the crossing at the time.
