Traffic has come to a halt on the QE2 bridge after a crash involving three vehicles.
Queues are stretching back as far as Junction 29 for A127 following the incident which happened between junctions 1A and 1B on the A282 in Kent.
A spokesman for South East Ambulance Service said: ""Just before 2:30pm we received a call to reports of a collision involving three vehicles on the Dartford Crossing.
"We sent out two ambulances and we took two people to hospital.
"They included an 18-year-old woman who was taken to Medway Maritime Hospital with neck pain, and a further patient, whose gender is unknown at this time, has been taken to Darent Valley Hospital with injuries unknown."
The incident occured at around 3:28pm and is ongoing.
