A SOUTHEND Hospital nurse is being brought before the Nursing and Midwifery Council for alleged misconduct.

Shagufta Nasreen Azhar, a nurse and deputy ward manager on the Southbourne Ward, is accused of nine counts of misconduct between September 4, 2011, and July 24, 2013.

She is accused of failing to administer 10mg rivaroxaban to a patient, or record why it was not administered on September 4, 2011; failing to administer Parkinson’s medication to another patient on October 20, 2011 and failing to administer doxycyline to a patient on March 3, 2012.

In addition, she is accused of failing to remove a pain relief patch from a patient before applying another patch on October 9, 2012, completing an inaccurate and/or incorrect audit of the weekly pressure ulcer register on June 23, 2013, and not completing more than one skin care bundle when it should have been completed twice in one day between July 7 and 8, 2013.

She is also accused of incorrectly completing acuity scores for patients on the ward between July 12 and 15, 2013, failing to complete and/or document an assessment on a newly admitted patient on July 12, and failing to demonstrate the safe administration of medication during a round on July 22 and July 24.

The hearing will take place between February 11 and 13 in Aldwych, London.
